Donation:	$10 $20 $30 $50 - Half-Century Level (I certify that I am under 40 years of age) $100 - Century Level $250 - JFK Level $500 - FDR Level Other: Other contribution amounts equal to the half-century level or greater will include the appropriate Century Club level membership. Century Club memberships run from July 1 through June 30 of the following year. Dues paid during June will be good throuogh the end of June the following year. The TOTAL of dues for a given Century Club level will be reduced by 50% during the period Jan 1 through May 31 and such membership shall expire on June 30 of the same year.

Membership Levels

Half-Century Level - $50 minumum annual contribution (those under 40 only)

Century Club Level - $100 minimum annual contribution

John F. Kennedy (JFK) Level - $250 minimum annual contribution

Franklin Delano Roosevelt (FDR) Level - $500 minimum annual contribution
